Personnel Forms
The appraisal forms below are provided in a Word format and should be completed as per instructions below. The forms are fillable, meaning that you may download the forms, fill them in, and then print them out.
INSTRUCTIONS:
- Forms must be printed front and back on one sheet
- Form must have original signatures when turned in to Personnel
- Form must be printed on color appropriate paper (available from our Supply Room) as follows:
